Celebrating 15 years with the Home Again Team

Marty O’Connor is a founding owner of Home Again Living and leads the company’s strategy, finances, and growth. Before helping establish Home Again, he worked in commercial real estate and banking, including as a senior vice president and regional director for a multinational bank. He has a BA in finance, real estate, and urban land economics from the University of Wisconsin–Madison. Marty also serves as president and managing principal of Realm Real Estate Development. Through his work with Home Again and local charitable organizations, he is committed to strengthening the communities the company serves.

Celebrating 15 years with the Home Again Team

Melissa O’Connor is a founding owner of Home Again Living and oversees the company’s daily operations, human resources, and regulatory compliance. She opened the first Home Again community as its Administrator and Community Director. Before helping establish Home Again, Melissa held a leadership role in managed care, developing expertise in Medicaid and Medicare enrollment and provider contracting. She works closely with team members across Home Again’s communities to support consistent, attentive care. Melissa also champions connections between residents and the wider community, helping make each location a welcoming place to live and work.

Celebrating 10 years with the Home Again Team

Brooke LaChance guides clinical care across Home Again Living’s communities. A nurse practitioner with 25 years of nursing experience in geriatric, pulmonary, critical care, and transplant medicine, she brings a broad clinical perspective to resident care and team leadership. Alongside her Home Again role, Brooke continues to work as a nurse practitioner with the pulmonary clinic at UW Hospital. Earlier in her career, she worked as a registered nurse in the UW Hospital intensive care unit. Brooke earned a Bachelor of Science in Nursing from Linfield College and a master’s degree in nursing from the University of Wisconsin–Madison. Celebrating 9 years with the Home Again Team

Shyla Boortz helps Home Again Living’s communities run smoothly while keeping residents and families at the center of operational decisions. She joined the organization as a college intern after six years of pharmacy experience and has since taken on growing leadership responsibilities. Shyla holds a Bachelor of Science in Healthcare Administration with a minor in Business Administration. Today, she works across teams on processes, compliance, and service improvements. Outside work, she enjoys Wisconsin’s outdoors with her husband.

Celebrating 14 years with the Home Again Team

Crystal Long leads staff training and compliance efforts across Home Again Living. She joined the organization in 2012 and brings extensive experience in assisted living, along with an associate degree in Health Care Administration and training in dementia care and challenging behaviors. Crystal developed Home Again’s Thrive Program, which supports people living with dementia through individualized approaches and staff education. She works with each community to help team members put thoughtful, consistent care into practice. Away from work, she enjoys time with her family, gardening, camping, and their hobby farm.

Celebrating 5 years with the Home Again Team

Danielle Beach supports medical teams across Home Again Living’s communities. She began caring for older adults as a certified nursing assistant in high school and later earned an associate degree in nursing. Her four years of nursing experience include medical-surgical and critical care settings. As a float medical manager, Danielle brings that range of experience to the communities she serves and works with colleagues to provide attentive, compassionate support to residents. Outside work, she enjoys time with family, friends, and her chocolate lab.

Angela Fitzgerald helps Home Again Living recruit the people who care for and support its residents. She holds a Bachelor of Science in Healthcare Management from Globe University and brings experience from several healthcare roles. Her understanding of healthcare teams informs how she connects candidates with opportunities across Home Again’s communities. Outside work, Angela enjoys traveling, sunny days, and time with family and friends.

Celebrating 6 years with the Home Again Team

Mike Matney helps keep Home Again Living’s buildings safe, comfortable, and ready for daily life. He brings experience in facility and nursing home maintenance and understands how dependable spaces support residents and staff. Mike works across a range of maintenance needs, helping each community function smoothly. Outside work, he enjoys traveling, visiting state parks, and cheering on Wisconsin sports teams with his family.

Celebrating 5 years with the Home Again Team

Robert Nakkula supports the upkeep of Home Again Living’s communities. With more than a decade of maintenance experience, he helps address facility needs that affect residents’ comfort and staff members’ work. Robert brings a practical, attentive approach to keeping the buildings running well. Outside work, he enjoys traveling and spending time on the water with his family, along with following the Packers and Badgers.
