Brooke LaChance guides clinical care across Home Again Living’s communities. A nurse practitioner with 25 years of nursing experience in geriatric, pulmonary, critical care, and transplant medicine, she brings a broad clinical perspective to resident care and team leadership. Alongside her Home Again role, Brooke continues to work as a nurse practitioner with the pulmonary clinic at UW Hospital. Earlier in her career, she worked as a registered nurse in the UW Hospital intensive care unit. Brooke earned a Bachelor of Science in Nursing from Linfield College and a master’s degree in nursing from the University of Wisconsin–Madison. Celebrating 9 years with the Home Again Team

Lauren Loofboro leads Home Again Living’s Cambridge community and brings 20 years of nursing experience as a licensed practical nurse. She began her career at a local family practice clinic, where she spent a decade caring for patients, including older adults. Lauren studied nursing at Madison Area Technical College. At Home Again, she works with residents, families, and care providers to help coordinate individual care and support the Cambridge team. Outside work, she enjoys family time and cheering on the Packers and Badgers.

Celebrating 8 years with the Home Again Team

Theresa Hetzel brings more than 30 years of nursing experience to Home Again Living’s Waunakee medical team. A licensed practical nurse, she previously spent 27 years at a local skilled nursing and assisted living community. Theresa studied nursing at Madison Area Technical College and draws on her hands-on background to work closely with residents, families, and primary care providers. She values relationships that help care reflect each resident’s individual needs. Outside work, she enjoys hiking, biking, canoeing, and traveling with her husband.

Celebrating 5 years with the Home Again Team

Danielle Beach supports medical teams across Home Again Living’s communities. She began caring for older adults as a certified nursing assistant in high school and later earned an associate degree in nursing. Her four years of nursing experience include medical-surgical and critical care settings. As a float medical manager, Danielle brings that range of experience to the communities she serves and works with colleagues to provide attentive, compassionate support to residents. Outside work, she enjoys time with family, friends, and her chocolate lab.

Alexandra Dickman brings experience across several senior care settings to her role as medical manager at Home Again Living’s Waunakee community. She has three years of nursing experience and began her geriatric care career as a certified nursing assistant, later worked in assisted living case management, and served as a director of nursing in skilled nursing. Alexandra earned a Bachelor of Science in Nursing from Herzing University and became a registered nurse in 2023. She works with residents, families, and colleagues to support coordinated, attentive care. Outside work, she enjoys gardening, boating, fishing, and time with her pets

Patti Purtell brings a strong background in healthcare and a passion for serving older adults to her role as Medical Manager at Home Again Columbus. She earned her Practical Nursing degree from Madison Area Technical College in 2010 and has built extensive experience across several areas of healthcare.

Patti began her nursing career in a General Surgery Clinic, where she worked for three years before transitioning into long-term care. She later spent five years working in skilled nursing facilities and another five years in assisted living, where she developed a deep appreciation for supporting the geriatric population.

Her journey in healthcare started even earlier, working in housekeeping and as a CNA in a skilled nursing facility, experiences that shaped her compassionate and hands-on approach to resident care.

Originally from Poynette and later raising her family in Pardeeville, Patti now lives in Sun Prairie. Outside of work, she enjoys dancing, shopping, tropical vacations, and spending time with family and friends. She and her husband of 29 years have five children and six grandsons.
